ICD-10: Multiple Gestation

When ICD-10 is implemented in October, 2015, a significant change for MFM will be the ability to assign a condition to a specific fetus in multiple gestation pregnancies. This is done using a 7th character on the ICD-10 code for certain categories.

The designations are:

0 = not applicable or not specified

1 = fetus 1

2 = fetus 2

3 = fetus 3

4 = fetus 4

5 = fetus 5

9 = other fetus

For single gestation, or multiple gestations where the fetus is not specified, the 7th character 0 is assigned.

An example of assignment of a condition to a fetus in ICD-10:

Di-Di twins, 24 weeks, 1 day - fetus 1 normal, fetus 2 with anencephaly

§  O30.042 Twin pregnancy, dichorionic/diamniotic, second trimester

§  O35.0XX2 Maternal care for CNS malformation in fetus, fetus 2

§  Z3A.24

Note that X is used as a placeholder for the 5th and 6th character, in order to apply the 7th character for the fetus. An additional code (Z3A.24) is used to specify the number of completed weeks gestation.
