Surveys of Interest to MFM's

Prepayment is required on all Survey of Interest orders. SMFM does not accept purchase orders or invoices. The total sum covers the cost of posting your survey for six (6) months, which must be reviewed and approved by the SMFM Survey Review Task Force. Approved surveys with complete payment will be posted on the SMFM Survey of Interest web page and accessible to SMFM members only.
