ICD-10 Coding for Injury/Accidents During Pregnancy
When a patient presents after having been involved in an accident, such as a motor-vehicle collision or fall:
> If the patient is being observed without symptoms, code O9A.21X, Injury, poisoning, and certain other consequences of external causes complicating pregnancy. This code should be followed by the appropriate code from Chapter 20, External Causes of Morbidity, and Z3A.XX for weeks of gestation.
Example: Patient at 24 weeks presents after being in a motor vehicle accident as a passenger in a car. She has no symptoms, and her evaluation is normal.

2. V49.9XXA Car occupant in unspecified accident, initial encounter
3. Z3A.24 24 weeks gestation
> If the patient is injured or exhibits symptoms, the Chapter 15 ICD-10 code corresponding to the symptom(s) or complication(s) should be used, along with any injury/symptom code(s), followed by the external cause code, and Z3A.XX for weeks of gestation.
Example: Patient at 33 weeks presents after falling in the shower. Evaluation reveals no pregnancy complications. She is found to have a sprained right wrist.
